First, make sure you clarify why you need the Ignition Interlock, whether it’s through the Florida Highway Safety and Motor Vehicles (DHSMV) department or the Diversion Program. You’ll also need to specify when your license revocation or suspension took place for Smart Start to properly set-up your account.
Once your preferred date and time is confirmed, make sure to bring the following with you to your installation appointment:
- Two forms of ID
- Insurance card
- Vehicle registration card
- If you are using someone else’s vehicle for your Interlock program, our service center agent will provide a “Florida Authorization to Install” sheet for the vehicle owner to fill out.
- Licensed driver’s statement
- This statement will be provided to you at the service center where it must be filled out and signed before leaving.
- A designated driver with you at installation
