Must have paid a Security Deposit
Paid Lockout date was advanced into the future 1 or more months
Device must be returned to Smart Start inventory and account must be closed, unless it is pre-approved by Smart Start
Valid Email Address and Phone Number must be provided

Disqualifications

Security Deposit was not paid
The device was not returned, returned damaged or not functioning
Past due balance exceeds the refund amount
If you have already disputed charges with your banking/financial institution

If you are not eligible for a refund, you will receive an email with information regarding the disqualification

The refund form should only be completed by customers who have completed their program and all devices have been returned resulting in a closed account. Any other disputes as stated above will be automatically rejected. Please read qualifications above.

If it is determined that a refund is to be issued, the refund will be issued in the form of a check delivered to the mailing address entered on this form.

Smart Start holds the right to cancel a dispute if a chargeback was issued to the original form of payment.
